(A PDF searchable from Google- difficult to acquire the URL though- gives US premiere info for the concerto- though it gives the mode minor- A minor- rather than A major. Edit: which is as well. it is in minor, after all!)

The Violinist, volume 14, October 1912 issue, page 37, mentions the "initial performance" of the concerto, October 3 1912, Berlin, with the Berlin Philharmonic and the same soloist (unspecified conductor).

Emery: The Violin Concerto mentions the premiere earlier in Danzig.
