Janáček's Works: A Catalogue of the Music and Writings of Leoš Janáček, p.13 gives composition beginning before Dec. 1894, finished by March 1903, revisions in Oct. 1903, and according to letters to friends of the composer, some other revisions made ca.1907, Feb. 1908 and Nov. 1915.

According to the Dover reprint, Kovařovic's edits are reflected in the 1917 UE edition.
Another 1917 publication/vocal score (Urbánek plate 89, I think) (admittedly also hard to find) was translated (into French I assume?) by Romain Rolland, who died in 1944 and whose translation therefore might be less copyrighted...
